Medicaid Managed Care

New York Medicaid is the federal/state program for individuals and their families, particularly children, pregnant women, the elderly and the disabled. Eligible individuals receive a Medicaid benefit card.

 

Neighborhood Health Providers (NHP) offers most Medicaid benefits. Benefits not covered by NHP are offered through Medicaid Fee-For-Service. Medicaid eligible persons will use their Medicaid benefit card for benefits covered by Fee-For-Service Medicaid (example: pharmacy). The NHP ID card will be used for all covered benefits.

  • Specialist Visits
  • Vision, Speech and Hearing Services

  • Diabetic Supplies and Monitoring
  • Family Planning/Reproductive Health
  • Hospital Outpatient Services and Clinic Visits
  • Radiation/Chemotherapy/Hemodialysis
